Fill out the delivery application completely, sign it, and submit it to Peabody s. You must be 21 years old, have a valid form of ID and a valid credit/debit card. Once you have applied, your application will be on file, so you can simply call Peabody s and place your order! | |

2. After you have placed your order with Peabody s, we will call Boone Take-Out Express with your order. The delivery driver will confirm the identity of the person who has placed the order upon delivery. Have your ID and credit/debit card on hand. | |

3. Your credit/debit card will be charged for your order plus the delivery charges by Peabody s. A copy of the credit card receipt will be included with your order and signed AS PER AGREEMENT. If you would live to tip the driver, let us know how much when you order. | |

To be able to receive beer or wine delivery you MUST have filled out a delivery application and submitted it in person at Peabody's and have your drivers license and Visa or Mastercard available for the delivery driver to confirm your identity.
